Contract Case Manager
Job Description

Location: Onsite |Â Phoenix, AZ

Duration of Contract:Â 4-6 Months
Â
Contract Rate: $24-$25 Hourly | Depending On Experience
Â
Important: This position requires COVID-19 vaccination (including boosters) AND Level One Fingerprint Clearance Card
Â
Background Profile:
- 2 or more years’ of Case Manager experience
- High School Diploma required | Bachelor’s Degree a Plus
- Exceptional Communicator & Listener that has a passion for Customer Service & helping others
- Coordinate and assist clients in navigating healthcare, housing and social service systems, which may include helping clients apply for benefits, schedule and attend appointments, provide in-person learning experience of navigating the public transportation system in the state and translation services
- Degree in Sociology, Social Work, Psychology, or Human Services preferred
- Proficiency in MS Office Suite including Excel & Word
- Bi-lingual is a plus but not necessary
- Must be a Permanent Resident – U.S. Citizen or Green Card holder to be considered
